Thickeners are one of the most common tools used to help people with swallowing difficulties drink more safely. Used at the right level and prepared consistently, they can make a real difference to swallowing safety.

This guide explains what dysphagia is, why thin drinks can be harder to swallow, when thickeners are recommended, what the IDDSI levels mean, and how to use thickeners properly at home, including the less obvious issues like hydration, medications and safe storage.

What is dysphagia and who does it affect?

Dysphagia (pronounced dis-fay-juh) is the medical term for difficulty swallowing. It covers problems at any point in getting food or drink from the mouth, through the throat and down to the stomach.

A normal swallow happens in stages. The mouth and tongue shape food or drink into a manageable mouthful, the throat moves it past the airway, and the oesophagus (food pipe) carries it down. During that middle stage, the airway closes off for a split second so nothing "goes down the wrong way". When muscles are weaker, slower or less coordinated, that timing can slip.

Dysphagia is more common than many people realise. In Australia, an estimated 15–30% of people over 65 have some form of swallowing difficulty.

Common causes

Dysphagia is usually a symptom of another condition rather than a condition in itself. Common causes include:

Stroke, which is one of the most common causes of sudden swallowing problems
Neurological conditions such as Parkinson's disease, motor neurone disease (MND) and multiple sclerosis
Dementia, particularly in later stages
Head and neck cancer and its treatment
Acquired brain injury
Age-related changes, as muscle strength and coordination can decline with frailty

Signs someone may be having trouble swallowing

Some signs are obvious, while others are easy to miss or put down to something else. Watch for:

Coughing, choking or throat clearing during or soon after eating or drinking
A wet or "gurgly" sounding voice after swallowing
Food or drink coming back up, or food left in the mouth after swallowing
Taking much longer to finish meals, or avoiding certain foods
Repeated chest infections
Unexplained weight loss
Avoiding social meals because of swallowing difficulty or fear of choking

The Aged Care Quality and Safety Commission lists similar red flags for aged care staff. If any of these sound familiar, it's worth raising with a GP, who can refer to a speech pathologist for assessment.

Why thin drinks can be harder to swallow safely

It surprises many people that water can be one of the hardest things to swallow when someone has dysphagia. Solid food at least holds together and moves slowly. Thin liquids move fast and spread easily, so they can reach the top of the airway before it has had time to close.

When food or drink enters the airway below the vocal cords, it's called aspiration. Often the body responds with coughing, which is the airway trying to clear itself. But aspiration can also happen without any obvious cough, particularly when sensation in the throat is reduced. This is sometimes called silent aspiration, and it's one reason a professional assessment matters more than simply watching for coughing.

How thickening helps

Thickening a drink changes how it behaves in the mouth and throat in two useful ways:

It slows the flow. A thicker drink moves more slowly, giving the swallowing muscles more time to coordinate and close the airway.
It holds together. A thickened mouthful tends to stay as one cohesive portion instead of scattering, which makes it easier to control.

Imaging studies of swallowing support this: thickened fluids have been shown to reduce aspiration and improve swallowing function in many people with dysphagia.

What the evidence does and doesn't show

The evidence has limits, though. While thickened fluids reduce aspiration during swallowing, systematic reviews have not found convincing evidence that they prevent pneumonia or improve longer-term outcomes on their own. The link between aspiration and pneumonia is complex and also depends on factors such as oral hygiene, mobility and overall health.

This doesn't mean thickeners don't work. It means they are one tool within a broader plan, and that the right approach is individual. That's exactly why a speech pathologist's assessment is the starting point.

When are thickeners recommended?

Thickened fluids are usually recommended after a swallowing assessment by a speech pathologist, the health professional who specialises in swallowing as well as communication. The assessment often starts at the bedside, watching how someone manages different drinks and foods.

Where more detail is needed, the speech pathologist may use an instrumental test such as a videofluoroscopic swallow study (VFSS) or a fibreoptic endoscopic evaluation of swallowing (FEES). A VFSS is a moving X-ray of the swallow. FEES uses a thin camera passed through the nose. Both show what's happening in the throat, including aspiration that can't be seen from the outside.

Not everyone with dysphagia needs thickened drinks

Thickening is only one option. Depending on the type of swallowing problem, a speech pathologist might instead or as well recommend:

Sitting fully upright for meals and for a period afterwards
Specific swallowing techniques or postures, such as tucking the chin during a swallow
Swallowing exercises to strengthen muscles
Smaller sips, slower pacing, or supervision during meals
Changes to food texture

The right level is according to an individual’s specific condition

When thickening is recommended, the speech pathologist will specify a level. More isn't automatically safer. A drink that's thicker than needed can be harder for some people to move through the throat, less pleasant to drink and more likely to be left unfinished.

Recommendations also change over time. Someone recovering from a stroke may move back towards thin drinks as their swallow improves, while someone with a progressive condition may need more support later. If you think the current level no longer suits, ask for a review rather than adjusting it yourself.

People also have the right to make informed choices about their own eating and drinking. In aged care this is sometimes formalised as Eating and Drinking with Acknowledged Risk (EDAR), where a person, their family and the care team agree on a plan that balances safety with what matters to the person.

Understanding IDDSI drink thickness levels

Since 1 May 2019, Australia has used the International Dysphagia Diet Standardisation Initiative (IDDSI) framework to describe thickened drinks and texture-modified foods. It replaced the older Australian standards, and it's supported by Speech Pathology Australia and Dietitians Australia as best practice.

The practical benefit is consistency. "Level 2" means the same thing on a hospital discharge letter, a product label, an aged care menu and in your kitchen.

IDDSI uses a scale from 0 to 7. Drinks use Levels 0–4, and each level has a number, a name and a colour code:

A note on older labels: under the previous Australian standards, drinks were labelled 150, 400 and 900, and mildly thick drinks had a green label. Under IDDSI, mildly thick is now pink and green means extremely thick. If you have older printed instructions or stock at home, check them against the current level rather than going by colour.

Checking thickness at home with the IDDSI Flow Test

It's hard to judge thickness by eye, and drinks can look right while being too thin or too thick. The IDDSI Flow Test gives a simple, objective check using a standard 10 mL slip-tip syringe:

Remove the plunger from the syringe.
Cover the nozzle with your finger and fill the syringe to the 10 mL line with the prepared drink.
Remove your finger and start a timer.
At 10 seconds, cover the nozzle again and read how much is left.
Compare the amount left with the table above.

Syringe sizes vary, so IDDSI specifies one where the 10 mL scale measures 61.5 mm long. For Level 4, IDDSI uses fork and spoon tests instead, because the drink is too thick to flow.

Aged care facilities and hospitals often use purpose-made IDDSI Flow Test funnels, which are pre-marked with the IDDSI levels, for staff training and routine checks.

IDDSI also covers food textures from Level 3 (liquidised) to Level 7 (regular). This guide focuses on drinks, but the speech pathologist may recommend a food texture level as well.

Types of food and drink thickeners

Thickeners aren't all the same, and the type affects how a drink looks, tastes and behaves over time. Most fall into one of four groups:

Why saliva matters

The biggest practical difference between starch and gum thickeners is how they respond to saliva. Saliva contains an enzyme called amylase that breaks down starch. When someone sips slowly, saliva mixing back into the cup can gradually thin a starch-thickened drink, so the last mouthfuls may be thinner than the first.

Xanthan gum isn't broken down by amylase. A 2021 systematic review found xanthan gum thickeners were more cohesive, more stable over time and temperature, and more resistant to saliva than starch-based products. It also found both powder and liquid xanthan thickeners improved swallowing safety, although the evidence for liquid forms is more limited.

Liquid thickeners

Liquid thickeners are a newer option. Because they're already a gel, they mix in without clumping and can be dispensed in measured doses, which can help with consistency when several people prepare drinks.

One example is Precise Thick-N INSTANT Liquid Thickener, which uses a precision pump to dispense a measured volume. According to the manufacturer, it reaches its prescribed thickness in about 30 seconds in most drinks and stays stable for up to 24 hours under recommended conditions. It's suited to home use as well as aged care and hospital settings.

Consistency matters more than brand

Whichever type you use, the most important thing is getting the same result every time. Different products need different amounts to reach the same IDDSI level, and scoops or pump doses aren't interchangeable between brands. If you switch products, check the new instructions and retest the result, and let your speech pathologist know.

How to use thickeners correctly at home

The exact method depends on the product, so always follow its instructions first. Most thickeners follow the same basic pattern:

Confirm the prescribed level. Check the IDDSI level on the speech pathologist's recommendation, not on an old label or memory.
Measure the drink. Use a measuring jug or a cup you know the volume of. The amount of thickener depends on the volume of liquid.
Use the product's own measure. Use the scoop or pump supplied with that product, and level scoops off rather than heaping them.
Mix as directed. Most products are added to the drink while stirring or whisking briskly. For hot drinks, add milk and sugar before the thickener.
Allow standing time if needed. Some thickeners reach their final thickness within seconds, others take longer. Starch-based products may keep thickening as they stand.
Check before serving. Look for lumps and, if you're unsure, use the IDDSI Flow Test.

Common mistakes to avoid

Guessing amounts or using kitchen teaspoons instead of the supplied scoop or pump.
Topping up a thickened drink with thin liquid, or adding more powder to a drink that has already thickened. If a drink is wrong, start again.
Assuming ice, ice cream or jelly are fine. These melt into thin liquid in the mouth. Check with the speech pathologist before including them.
Forgetting about soups, sauces and gravies. Thin liquids at mealtimes may need thickening too.
Shaking carbonated or hot drinks to mix them. Stir instead, and check the product's guidance for fizzy drinks.
Preparing starch-thickened drinks too far ahead, as they can become thicker than intended. Check the product's guidance on how long prepared drinks remain stable.

Store thickener safely

This one is easy to overlook. Dry thickening powder can form a solid mass if swallowed on its own, which can block the airway. In 2015, NHS England issued a patient safety alert after a care home resident died following accidental ingestion of thickening powder left within reach.

The risk is highest for people with dementia, confusion or poor vision. Keep tubs closed and stored out of reach, ideally somewhere you'd keep medications, rather than on the bedside table or dining table.

Hydration, medications and enjoyment

Getting the thickness right is only part of the picture. The other part is making sure the person actually drinks enough, takes their medications safely and still enjoys what they eat and drink.

Keeping up fluid intake

People on thickened drinks often drink less. Thickened fluids can feel more filling, taste different, and simply be less appealing than a glass of water. Research has linked thickened fluids with reduced fluid intake, and in one large trial, dehydration was more common among people given thickened liquids than those using a posture technique instead.

Practical ways to help:

Offer drinks little and often through the day rather than relying on mealtimes.
Keep a prepared drink within easy reach, at the right level.
Vary flavours and temperatures, within what the speech pathologist allows.
Count other fluids, such as thickened soups, smoothies and yoghurt-based drinks.
Keep a simple fluid chart for a few days if you're unsure how much is being drunk.

Watch for signs of dehydration, including dark urine, a dry mouth, headaches, dizziness, constipation, or new or worsening confusion. These are worth raising with a GP.

Medications and thickened fluids

Swallowing tablets is often harder than eating, and the drink used to take them may need to be thickened too. This needs a conversation with a pharmacist or GP, for a few reasons:

Thickeners can affect how some medications work. Studies have found that thickened fluids can slow how quickly some medicines dissolve, particularly at higher thickness levels.
Some combinations don't mix well. For example, macrogol laxatives can break down starch-based thickeners and turn a thickened drink thin again.
Crushing or opening medications isn't always safe. Some tablets are designed to release slowly or protect the stomach, and crushing them can change how they work.

A pharmacist can suggest suitable forms, such as liquids, dispersible tablets or patches, and explain how to give each medication safely. It's also a good opportunity to review whether every medication is still needed.

Keeping drinks enjoyable

A thickened drink that someone dislikes is one they won't finish. A few things can help:

Gum-based and liquid thickeners usually keep drinks clearer and closer to their original taste than starch-based powders.
Try a variety of drinks, including flavoured milks, juices, cordials and favourite hot drinks.
Serve drinks in a familiar cup or glass rather than a medical-looking beaker where possible.
For outings, single-serve options or a small, sealed supply of thickener make it easier to keep socialising.
